Got the collectors edition (PC) on preorder from Amazon. I'll let you know when it turns up.

It's at the very end of the first mission, in the pillar with the blue glow coming from inside it near the far side of the lift platform, just behind the ammo crates. Stand next to the pillar and press O to swap your chainsword for the axe.

I just opened my copy of the collectors edition. Its really nice, and almost feels worth the extra cost.

Film called "pit and pendulum" starring him, where he is a sadistic witchhunter, controlling a cabal of warriors and wealthy lords who molest and burn women... for a devout crazy inquisitor..well he already done it (in film he has shaved head and tells his underlings to whip him everytime he's aroused by a woman)
(really quite a messed up film)
